＜症例報告＞強皮症と内臓悪性腫瘍

抄　録：18年来の経過を有するPSSに胃癌を伴った興味ある例を報告するとともに，両疾患の合併につき検討した。症例は60歳女。42歳時レイノー症状で発症。漸次皮膚の硬化，呼吸困難を呈した。現症として顔面・躯幹・前腕皮膚の硬化・褐色色素沈着・光沢，また指尖の潰瘍などを認めた。検査成績では，血沈値の亢進，軽度の貧血，拘束性呼吸機能障害，CRP(2+)，抗核抗体陽性，抗Scl-70抗体陽性，X線学的に肺の粒状網状陰影，食道下部の拡張と胃角部の陥凹性病変，歯根膜腔の開大，手指末節骨の吸収像および皮下石灰沈着などがみられた。硬化部皮膚の病理組織学的所見では結合織の膨化均質化，附属器の減少などがみられた。一方，切除胃の病理診断はsignet ring cell carcinomaであった。PSSに悪性腫瘍を合併した報告はこれまで極めて少ないが，一般に，両疾患の合併は単なる偶発とするより頻度が高いと考えられている。

Scleroderma and Internal Malignancy

A 60-year-old woman with progressive systemic sclerosis associated with gastric cancer is reported. She developed cancer after an eighteen-year history of scleroderma. Histological examination of the excised stomach specimen revealed signet ring cell carcinoma. Reports of the coincidence of malignancy and scleroderma have been inconclusive. Although the incidence of cancer with scleroderma is less than that with dermatomyositis, many investigators believe that the relationship is more than coincidental.
