Day Care Treatment for elderly Persons with Dementia

- Evaluation of Prognosis with respect to Physicak Activity and EEG -

Abstract: In elderly persons with dementia who live at home and exhibit troublesome behavior, clinical improvement is often observed with day care treatment (DGT). However, few studies have examined the treatment effects and mechanism. Since DGT is labor intensive, it is necessary to select patients who would optimally benefit from the treatment. However, no predictors are available to facilitate the selection of patients who will most likely receive benefit from DGT. In this study, DGT wa provided to 20 patients with dementia, and its effects on clinical symptoms were predisely evaluated, and physically activity and basal rhythm of electroencephalogram (EEG) were quantitatively analyzed. The subjects were randomly divided into two groups: "Improved group" (13 patients) and "Not-improved group" (7 patients), based on changes in clinical symptoms and physical activity after three months of DGT. In the "Improved group", troublesome behavior was disminished and night-time physical activity was reduced. After the first day of DGT, changes in basal rhythm of EEG were most marked in the α2 band. Wave % Time in the α2 band increased in the "Improved group", and decreased in the "Not-improved group". Conversely, inthe θ1 band, there was a decrease in the "Improved group" and an increase in the "Not-improved group". This suggests that analysis of basal rhythm of EEG on the first day of DGT can be a predictor of treatment effects three months later.
