＜症例報告＞重複長掌筋の一例

抄　録：1995年度の山梨医科大学肉眼解剖実習において，86歳，男性（急性心不全により死亡）の右前腕に二本の長掌筋が平行して並ぶ重複長掌筋が見られた。重複長掌筋は右上腕骨の内側上顆を起始として，撓側長掌筋とほほ同じ大きさの尺側長掌筋からなる。撓側長掌筋は下走しながら，筋腹から腱に移行し，屈筋支帯の上を超え手掌腱膜に終る通常見られる長掌筋であり，正中神経の支配を受けていた。尺側長掌筋は前腕下部で屈筋支帯に放散し，尺骨神経と正中神経の二重神経支配を受けていた。さらに尺側長掌筋は屈筋支帯より近位12mmの位置から異常筋が生じ， この筋束は小指外転筋と共に小指基節骨底に停止していた。長掌筋はヒトでは退化的な筋と考えられ，変異に富む筋の一つである。このたびこの重複長掌筋の起始と停止の観察，計測を行い，神経支配を観察したことを報告する。

A case of the Duplicate Palmaris Longus Muscle

A case of the duplicate palmaris longus muscle was found in the right forearm of a Japanese male cadaver of 86 years old. The ulnar element arose from the medial epicondyle of humerus with double innervation from median and ulnar nerves, and inserted into the flexor retinaculum. An aberrant muscle bundle arose from the tendon of the ulnar element at proximal 12mm of flexor retinaculum. This aberrant muscle together with abductor digiti minimi inserted upon proximal phalanx of the fifth finger.
It is considered that palmaris longus muscle is a regressive muscle in man among mammals with abundant variations. We report here the origin, insertion and innervation of a case of the duplicate palmaris longus muscle.
