外傷性四肢骨幹部骨折に対する創外固定器の治療成績

要 旨：髄内釘など内固定法の適応が困難なため初回治療として創外固定法を施行した多発外傷151例を含む外傷性四肢骨幹部骨折387例432骨折の治療成績から本法の骨折治療における有用性を検討した。骨折部位別では大腿部180骨折，下腿部（全例脛骨）が185骨折，上腕部と前腕部が各々43骨折と23骨折であった。このうち開放性骨折は285骨折と多数を占め，また創外固定器を複数の部位に装着した症例は33例78骨折であった。治療成績では，創外固定法単独で骨癒合を得る一期的骨癒合率は，大腿部の90.6％から上腕部の93.5％の平均92.1 ％，骨癒合期間は下腿部16.9週から前腕22.8週の平均18.7 週であった。創外固定法に起因する合併症は，ピン周囲の皮下浅層感染が11.0 ％と最も多かったが，骨髄炎に進展した症例はなく，創外固定抜去後に全例治癒している。ついで偽関節および遷延治癒骨折で，これらは骨移植術や内固定術が施行され，全例骨癒合をみていた。
　以上より創外固定法は骨折治療法の1 つとして有用である。

Clinical Results in the Traumatic Fractures of Diaphysis of the Long Bone Treated with External Fixator

Abstract: We reviewed the clinical results of 432 traumatic fractures in 387 patients by external fixation between 1984 and 2000. The sites of involvement were as follows: tibia,185; femur, 180; humerus, 43; and forearm, 23. The average rate of bone union was 92.1 ％and average period for bone union was 18.7 weeks by using only external fixa-tion. The complication was frequently superficial pin tract infection. The treatment of external fixation is one of a useful cures for fractures, and we are going to treat various fractures by external fixation in the future.
