Free Ball Thrombus of The Left Atrium

Abstract: Left atrial ball thrombus is a rare disorder. We report a case of a free ball thrombus in the left atrium complicating severe mitral stenosis. A 70-year-old woman had cerebral embolism followed by mild left hemiparesis. Two-dimensional echocardiography revealed a large left atrial thrombus, mitral valve stenosis and regurgitation and aortic valve stenosis and regurgitation. We decided to remove the ball thrombus and replace the mitral valve under the extracorporeal circulation, but she developed an attatk of diffuse abdominal pain. An angiography revealed the occlusion of the common hepatic artery and proximal superior mesenteric artery, and suggested severe intestinal necTosis. Thrombo-embolectonly of the superior mesenteric artery was performed, but she died of declamping shock during the thromboembolectomy and before the cardiac operation. At autopsy, the mitral valve deformity was severe and its orifice was slit-like, 0.5 cm in width, but aortic valve deformity was mild. The left atrium contained a ball thrombus with a concaved center without attachment to the atrial wall, 12 grams in weight and 3.5×3.0×2.0 cm in dimension, with lamellar cut surface.

左房内球状血栓症

抄　録：最近心エコー法の進歩により左房内血栓の診断が容易となってきているが左房内球状血栓ほ稀である。私たちは高度僧帽弁狭窄に合併した左房内浮遊球状血栓症を経験したので報告する。症例は70歳，女。30歳頃より心疾患を指摘されており時々息切れを自覚していた。脳塞栓による片麻痺が発症い断層心エコー図にて左房内球状血栓を発見され当科ヘ紹介された。当科入院時のカラードップラー法併用断層心エコー図にて高度僧帽弁狭窄兼閉鎖不全及び軽度大動脈弁狭窄兼閉鎧不全の合併を診断されたが，高齢でもあり手術適応決定の為入院3日目に心カテーテル検査を施行した。心係数1.6，肺動脈酸素飽和度57%と低く，右腎動脈完全閉鎖も認め，手術死亡率は高いと思われたが肺動脈圧26/10 (mean15)mmHg，LVEF0.53であり手術適応内と判定，準緊息手術を予定した。しかし翌日，腹腔動脈及び上腸間膜動脈の塞栓症を発症，さらにその血管造影施行中に脳塞栓を起こし意識消失した。広範な腸管壊死が危ぶまれた為，緊急で上腸間膜動脈血栓摘除術施行したがその術中より血圧が低下い開心術の機を逃して死亡した。断層心エコー法のみの診断で緊急手術を施行すべきであったとも反省させられる症例である。
