原発性肺癌の気管支擦過細胞診の検討
―病理組織学的診断との対比を中心に―

抄 録：山梨医大附属病院開院後4年間に行われた気管支擦過細胞診による肺癌組織型の判定と病理組織学的診断（組織診）におけるそれとの対比検討を行った。組織診の裏付けのある気管支擦過細胞診63例の中，両者の組織型の一致をみたもの54例(85.7%)で，残りの9例が不一致であった。9例の内訳をみると組織学的に扁平上皮癌と診断されたもの(4例)を細胞診で腺癌(3例)または小細胞癌(1例)にしたものおよび組織学的に腺癌(5例)であったものが扁平上皮癌(3例)あるいは大細胞癌(2例)とされたものであった。不一致の要因を検討すると，(1)組織診と細胞診では，組織片と個々の細胞という検索対象が異なるため癌の組織型の決め方に相違があり，前者では細胞型と腫瘍構築より，後者では個々の細胞型より判定していること，(2)肺癌の組織像は他臓器の癌と比較して多彩であるために，組織の一部だけを採取して診断する生検およぴ気管支擦過細胞診では腫瘍全体の細胞型をカバーできないこと，(3)気管支擦過細胞診の新鮮細胞による組織型の推定は従来の喀痰における自然に剥離した変性細胞と比較し，組織型の差による細胞の特徴に乏しいこと，(4)扁平上皮癌とも腺癌とも判定し難い例がwaste basket的に大細胞癌として分類されることが少なくないことなどである。

A Cytopathologic Study of Bronchial Brushing on Primary Lung Cancer

A cytopathologic study of bronchial brushing was compared with histopathologic diagnosis in 63 cases of primary lung cancer at Yamanashi Medical College Hospital during four years. In 9 out 63 cases, cytopathologic diagnosis was not in concurrence with histopathologic diagnosis as follows; four cases of squamous cell carcinoma were cytologically classified into adenocarcinoma (three cases) and small cell carcinoma (one case), and five cases of adenocarcinoma into squamous cell carcinoma (three cases) and large cell carcinoma (two cases). The discrepancies between cytopathologic and histopathologic diagnosis were mainly due to differences in the method of classification, histologic variations of primary lung cancer, poor characteristics in cytologic features obtained from bronchial brushing and 'waste basket' classifying into large cell carcinoma.
