＜症例報告＞基底細胞母斑症候群の1例

抄　録：基底細胞母斑症候群 basal cell nevus syndromeは，皮膚の多発性基底細胞母斑，上皮腫，種々の骨格系統の異常を呈する症候群であるが，特に口腔領域では多発性顎骨嚢胞が特徴的な疾患である。今回，我々は，家族性に発症したと思われる本疾患に，ハイドロオキシアパタイト骨補填材を用いて顎骨嚢胞を中心とした治療を行い，良好な結果を得ているので，その経過とともに，本症についての若干の考察を述べる。症例は，15歳女性で，主訴である左上顎臼歯部の自発痛から多発性顎骨嚢胞が発見され，精査の結果，上記疾患と診断された。その他の症状は，皮膚では，両手掌，両背に点状小窩を呈する pitが認められ， また，骨格系については，肋骨の2分化，脊柱の側彎，後彎を認めた。また， レントゲン像上で，左側の上，下顎骨と右側の上顎骨に何れも埋伏歯を含む顎骨嚢胞が認められた。そして，本報告例では，家族性発症が疑われ，母と姉のみ精査可能で，その結果，本疾患と診断された。当科における治療は，顎骨嚢胞に対するものであった。顎骨襲胞の治療として，大きな嚢胞については，従来，開窓療法が行われているが，アパタイトの補填と一次閉鎖により，顎骨欠損，変形を後遣することなく，一期的に治癒し得た。

A Case of Basal Cell Nevus Syndromc

Basal cell nevus syndrome consists of multiple basal cell nevi, epithelioma and various abnormalities of the skeletal system. Particularly in the oral and maxillofacial region, multiple jaw cysts are a characteristic finding of this syndrome. We reported a familial case in which jaw cysts were successfully treated with hydroxyapatite ceramics. The patient was a 15-year-old female with a complaint of pain around the molars of the left maxilla. Examination revealed multiple jaw cysts with impacted teeth in the upper and lower jaw. Basal cell nevus syndrome was diagnosed on the basis of study. There were pits in both palms and on the back of the hands as skin signs. In the skeletal system, there was adhesion of vertebrae thoracales and vertebrae cervicales and scoliosis and hyposis of the spinal column. Although marsupialization is a conventional treatment of giant jaw cysts, the use of apatite made it possibIe to cure the disorder in one step with no residual defect or deformity of the jaw.
