Changes in Blood Transfusion during Liver Resection over the Past Ten Years

Abstract: Over the past 10 years, circumstances of blood transfusion during liver resection have changed in Yamanashi Medical University Hospital. After establishment of the blood supply system by the Red Cross Blood Center and a decrease in blood loss during liver resection in patients with liver cirrhosis (LC), we decided in August 1993 to stop preparing donor blood for elective liver surgery. Causes of the change were demonstrated by comparing the type of blood transfusion between the 37 patients [20 with LC, and 17 without LC] who underwent liver resection since October 1983 (first period) and the 37 patients [20 with LC, and 17 without LC] who underwent liver resection before August 1993 (second period). In the first period donor blood was more often used for both LC and non-LC patients. But it was not used in the second period in LC patients who had less liver dysfunction and more often underwent minor liver resection for small tumors with less blood loss (P<0.05). On the other hand, the non-LC patients of the second period, who underwent more complicated liver resection with bile duct reconstruction, had longer surgery duration and no reduction of the blood loss. However, no coagulopathy occurred in these cases even after massive bleeding and whole blood transfusion from tht Red Cross Blood Center. Preparation of donor blood is no longer necessary for recent elective liver surgery.
