A case of Pneumocystis Carinii Pnuemonia in an AIDS patient Diagnosed by decreased CD4/CD8 ratio of Bronchoalveolar lavage Fluid

Abstract:This case is 44-year-old man who was admitted with recurrent fever, non-productive cough, and dyspnes. Chest X-ray and CT showed bilateral diffuse ground glass opacity with peripheral clear zone. Hypersensitivity pneumonitis, Bronchiolitis obliterans organizing pneumonia (BOOP), Drug-induced pneumonia were considered as differential diagnoses. Cellular analysis of Bronchoalveolar Lavage Fluid (BALF) reveald CD4/CD8(T-cell) ratio markedly decreased:0.03. Pnuemocystis carinii pneumonia was diagnosed by Transbronchial lung biopsy (TBLB). A serological test for human immuno-deficiency virus(HIV)-1 was positive. This case and other reports suggests that markedly reduced CD4/CD8(T-cell) ratio in BALF indicates PCP in AIDS patients.
