訪問看護における在宅療養者・家族の自己決定と支援に関する研究
―療養者・家族間で意思が異なる場面の分析結果を中心に―

　本研究の目的は，療養者・家族の自己決定を支援する訪問看護婦の援助姿勢と方法を明らかにすることである。２８名の訪問看護婦に対し面接調査を実施し，療養者・家族の意思が異なる場面に焦点をあてて分析した。その結果，訪問看護婦の援助姿勢の特徴は「療養者・家族間で決定することが基本である」，「療養者の意思を尊重する」，「家族，特に介護者の意思を優先する」の３カテゴリーが抽出された。各々に含まれるサブカテゴリーには，療養者・家族の意思の“揺らぎ”とその支援に関連する共通の要素が見いだされた。これらは，主に５年以上の訪問看護経験を有する訪問看護婦に認められたことから，自己決定の支援技術の中でも高度なものといえるが，援助方法には課題が残されていた。訪問看護婦は，彼らの“揺らぎ”に影響されて自らも動揺し，精神的に不安定な状態にあった。訪問看護婦に対する情緒的サポートを含めて，システムとしてのバックアップ体制を整備する必要性が示唆された。

A Study of the Visiting Nurse's Support for Self-decisions Made by the Patient and Patient's Family
―Focused on Cases Where Both Parties Make Different Decisions―

　The purpose of this study was to define the supporting attitude and methods of visiting nurses who support self-decisions made by the patient and patient's family.
　We interviewed 28 visiting nurses and analyzed specifically the cases where both parties made different decisions.　As a results, we extracted three categories from the characteristics of the mental attitude of visiting nurses: (a)Decisions should basically be made between the patient and family, (b)Respect their patient's will, and(c)Prioritize the family's will,especially the caregiver's.　And we found that the sub-categories in each category have common elements related to the "wavering" in the patient's and family's will and the nurse's support for it.Since these elements were found in nurses who have five or more years of experience, they are higher skills among the supporting skills for self-decision.　Yet there was a problem left in their supporting methods.Visiting nurses also wavered in will by the influence of patient's and family's wavering,and therefore, their mental condition was unstable.
　It was suggest that we need to develop a backup system that also includes the mental support for visiting nurses.
