老人保健施設の入所者の平均入所期間と関連している要因

抄録
　老人保健施設の入所者の平均入所期間に関連している要因を明らかにすることを目的とした。研究の主旨に同意の得られた146施設のうち有効回答した104施設を分析対象とした。従属変数を施設の入所者の平均入所期間とし，独立変数を併設施設の有無，100床あたりの看護・介護職員数，看護婦・介護福祉士の有資格者の割合，入所者の痴呆・ADLレベル，施設の家庭復帰率として重回帰分析をおこなった。
　特別養護老人ホーム併設，訪問看護ステーション併設，在宅介護支援センター併設，100床あたりの看護職員数，100床あたりの介護職員数，介護職員に占める介護福祉士の割合，入所者の痴呆の程度，総退所者にしめる家庭復帰した者の割合は，施設の入所者の平均入所期間に関連していた（寄与率：49.1％）。
　看護職は平均入所期間の長い施設や入所者の痴呆が重度の施設において多く配置され，施設の家庭復帰率を高める役割を担っていることが示唆された。

The factors related to the average term of the residents' stay
in geriatric health service facilities

This research aimed at clarifying the factors which relate with the average term of residency in geriatric health service facilities.
Cooperation for investigation was obtained from 146 facilities.Out of the 146 facilities,104 had effective data and were analyzed using multiple regression.
In multiple regression,the dependent variable was the average term of the residents'stay in geriatric health service facilities.The independent variables were grades of a person's dementia,grades of a person's ADL,facilities in conjunction with hospitals,facilities with special nursing homes,facilities with home care service support centers, facilities with visiting home care stations,the number of nursing staff per 100 beds,the number of care workers per 100 beds,the ratio of registered nurses to nursing staff,the ratio of certified care workers to care workers,the ratio of patients returning home to patients discharged from the facilities.
The multiple regression model showed that facilities with special nursing homes,facilities with home care service support centers,facilities in conjunction with home care service support centers,the number of nursing staff per 100 beds,the numbers of care worker per 100 beds,grades of a person's dementia,and the ratio of patients returning home to patients discharged from the facilities contributed to the average term of residents'stay in geriatric health service facilities(coefficient of determination:0.49,P<0.0001).
This study suggests that more nursing staff is placed at the facilities where the residents stay longer or the residents have serious dementia.
It also suggests that the nursing staff performed a role in increasing the ratio of residents returning home to those discharged from the facilities.
