肝臓移植
―その現状と問題点―

要旨：臨床的に始められて25年が経過した肝臓移植は、手術手技の向上と強力な免疫抑制剤の出現によって最近では良好な成績が得られるようになり、様々な末期肝疾患に対する有効な治療法として認められつつある。しかし肝臓移植には、臨床上に限ってもいまだ多くの課題が残っていることも事実である。本稿では、欧米の施設からの最近の報告と、西ドイツ肝移植チームに加わった際の経験から、肝硬変症に対する手術時期、急性肝不全への応用、肝悪性腫瘍に対する対応、ドナー肝の確保、肝部分移植、胆道系合併症、肝動脈血栓症など、適応・臓器提供・手術手技の面で現在問題となっている諸点について報告する。本邦でも近い将来、肝臓移植が臨床的に開始されると推測されるが、そのためにも様々な問題点を通して欧米における肝臓移植の現状を理解しておくことが必要である。

The Present Status of Liver Transplantation

Twenty-five years have passed since the first successful liver transplantation was reported by Dr.Starzl. The refinement of the surgical technique and the discovery of more effective immunosuppressive agent have led to an improvement in results of liver transplantation, and now it has become the accepted mode of therapy for the patients with end - stage liver diseases. But it is the fact that even now not a few problems are remaining in some aspects of liver transplantation. In this article, we discuss several problems regarding patient selection, organ procurement, and surgical complication, such as the appropriate time of operation in cirrhotic patients, indication for malignancy, segmental transplantation, thrombosis of the hepatic artery, on the basis of the recent reports from Pittsburg, Cambridge, and Hannover. For the day of the commencement of liver transplantation in our country, which is supposed to be near at hand, it is very important for us to comprehend the present status of liver transplantation in Western countries.
