喉頭全摘出術患者のコミュニケーション状態認識の特徴
―術前後の変動・看護師の認識との相違―

喉頭摘出術患者のコミュニケーション状態の認識の術前から術後の変動と，看護師の認識との相違から，患者のコミュニケーション状態の特徴を明らかにし，患者が術前からの意思表出を低下させないための看護師の関わりを検討することを目的とした。喉頭摘出術患者7名に，SECEL尺度を用いて術前，術後7日目，術後1ヶ月目の3 回調査を行った。看護師の認識との相違をみるため，患者の担当看護師にも調査を行った。患者のコミュニケーション状態の認識は，術前より術後7日目で低下し，1ヶ月目には対人関係に関する項目が更に低下した。患者は看護師が認識している以上に会話に困難を感じ，他者との人間関係に支障を感じていた。看護師は，術後7 日目前後は身体的苦痛の緩和に加え，もどかしさへの共感，コミュニケーション方法の工夫，術後1ヶ月目前後には，対人関係への不安の軽減を念頭に置き，患者が自信を持って社会復帰できるような関わりが必要である。

Characteristics of Communication Perceived by Patients with Laryngectomy

In order to investigate nursing methods for preventing postoperative reductions in thought expression among laryngectomees, we studied the characteristics of the communication of patients with laryngectomy based on changes in the patients’ perception of their communication before and after surgery, in addition to differences in the nurses’ perception. A study using the Self-Evaluation of Communication Experiences After Laryngectomy (SECEL)scale was conducted on seven patients with laryngectomy a total of three times: preoperatively, seven days postoperatively, and one month postoperatively. The study was also conducted on the nurses in charge of the patients in order to assess differences in perception. The perceived communication of patients decreased from preoperatively to seven days postoperatively, and although they slightly improved one month postoperatively, items related to interpersonal relationships decreased further. Patients experienced more difficulty with conversations than was perceived by nurses, and had difficulty with interpersonal relationships. Nurses must provide care that enables patients to gain the confidence to reintegrate into society by alleviating physical pain, sympathizing with the patients’ frustration, and improving communication methods around seven days postoperatively, as well as considering the patients’ concerns regarding interpersonal relationships around one month postoperatively.
