手術を受ける患者の術前後における不安の変化
－STAI （日本語版）を用いて－

　過去の研究結果１）～２）から，手術室看護師が術前訪問を行うことは，術前の患者に有効であり，安心感につながることがわかっている。一方では，患者の信頼を得るには時間的な不足や看護師のコミュニケーション不足があり，手術室看護師の姿勢には患者の気持ちを考える意識や患者を尊重する行為が薄い現状が明らかになった。術前に，患者がどのような不安状態にあったかは,術後の回復に影響を及ぼす状態として問題になる３）という報告があることから，本調査の目的は，手術を受ける患者の術前後の不安の変化を知ること，また，患者の不安を軽減する有効な術前訪問の資料とすることとした。調査方法は，Y大学附属病院において，全身麻酔に硬膜外麻酔を併用し開腹術を受けた患者20名を対象とした。術前後の不安の変化はSTAIの日本語版６）を用い，手術の成功の認知はVAS法を用いて，無記名式一日留め置き法により調査を実施した。術前後の不安はSTAIの評価法に従い，術前後の不安の変化はｔ検定・手術の成功の認知はVAS法により評価後，術前後の関係を分析し，統計ソフトはSPSSを用いた。その結果，①術前の特性不安と状態不安は正の関係があった。②術前に手術の成功を信じている患者は，術後も手術の成功を認めていた。③術後に状態不安を強く感じている患者は，手術の成功の認識が薄いことが明らかになった。

Evaluation of perioperative anxiety in patients undergoing operation using　
STAI （Japanese Version） method

　Previous reports proved that pre-operation visits to patients by OR （Operating Room） nurses prevents patients from anxiety for operations. In addition, the lack of communications between patients and OR nurses is one of the reasons for patients' unsatisfactory feelings to OR managements. The objective of this study is to know perioperative patients' worries to operations and to get the effective information to pre-operation visits to patients. Twenty patients undergoing open abdominal surgery under general plus epidural anesthesia at Yamanashi University Hospital are studied. We evaluated the changes of patients' anxiety pre- and post-operatively with VAS and STAI method. We concluded as follows; 1） Pre-operative specific anxiety correlated well with status anxiety. 2） Patients believing operative good results preoperatively had a tendency to find operation succeeded. 3） Patients who were afraid of surgery preoperatively tended not to accept the success of operation
